Responsibilities:
- Provide telephonic nursing care and support to patients, addressing their concerns and medical inquiries.
- Assess patient conditions based on reported symptoms and provide appropriate recommendations.
- Educate patients on chronic disease management, medication adherence, and preventive care.
- Collaborate with healthcare teams to coordinate patient care and follow-up.
- Document all patient interactions accurately in the system.
- Triage patient concerns and escalate cases as needed to the appropriate medical professionals.
- Maintain patient confidentiality and comply with HIPAA regulations.
- Support patient engagement and adherence to treatment plans.
- Participate in ongoing training to stay updated on best practices in telehealth nursing.

Requirements:
- Active RN license in the state of practice.
- Minimum of two years of nursing experience, preferably in a telehealth, call center, or triage setting.
- Strong communication and active listening skills.
- Ability to assess and address patient concerns effectively over the phone.
- Proficiency in electronic health record (EHR) systems and documentation.
- Familiarity with chronic disease management and patient education.
