Job Responsibilities: Conduct telephonic clinical assessments using approved medical protocols Provide recommendations for appropriate levels of clinical care based on clinical judgment and protocols Outreach and engage members with programs to promote health and well-being Complete clinical eligibility assessments and identify health, condition, or social gaps Work Schedule: Monday-Friday: 2 shifts Pacific Time Zone between 8:30 am - 5 pm, then 3 shifts Pacific Time 10:30 am - 7 pm. Quarterly Saturday shifts 8:30 am - 5 pm Pacific time.
